This was an intumescent cataract with a not well-dilated pupil. Two stage capsulorhexis was planned, the small rhexis was successful. Decompression of the capsular bag is done by aspirating liquified lens matter by the Simcoe canula. Second Stage capsulorhexis was tried but capsulorhexis ran out. I don't want to endanger the patient's prospect of good vision ,so the procedure was converted to safer MSICS. The Tunnel was made and the nucleus was taken out the wire vectis. The IOL was implanted and one astigmatism correcting 10-0 nylon suture applied at the sclero-corneal tunnel.
